Student Program Requirements:

• Enrolled in required certification/degree program for student externship.
• Completion of necessary courses and curriculum prior to clinical rotation.
• Maintains appropriate licenses, certificates, and permits necessary for rotation.

Compliance Requirements:

• Proof of immunity to rubella and rubeola (two doses of MMR vaccine or serological testing).
• Proof of varicella immunity (verbal or written history or titers).
• Hepatitis B series initiation or declination and/or serologic testing.
• Two-step TB Skin test or Quantiferon testing within the last 12 months.
• Completion of flu shot during flu season.
• American Heart Association (AHA) approved BLS Certification, if applicable.
• Completion of HIPAA Compliance Training.
• Training in Human Blood and Body Fluid Precautions and Infection Control.
• Completion of criminal Background Check.
• Proof of a negative drug screen.
