Job Description:
Sherwood Pines is a small 16 bed memory care communitylocated in Veneta, Oregon. Hours are 11pm-7:15am. Veneta is just a short 15-20 minutes from West Eugene. The additional weekend differential is also available. Please also visit our website www.SherwoodPines.com to fill out the application.

Responsibilities:

• Must be able to pass a background check and complete preservice dementia, preservice infection prevention, and caregiver training.
• Treat all community members with respect, dignity, and compassion.
• Support and perform care as specified in the community member service plan.
• Assure community members are accounted for.
• Observe community members for changes in condition and report any significant changes.
• Respond to emergencies according to current policies and procedures.
• Completed incident reports per policy on assigned shift.
• Take and record vitals into community members' charts.
• Promptly respond to calls and/or alarms.
• Tidy community members' rooms and other common areas around the facility.
• Change linens and remove garbage.
• Complete laundry and ensure it is delivered back to the room promptly.
• Transfer and escort community members following care plans and safe practices.
• Document care was provided.
• Read and contribute to shifting reports and care notes to ensure continuity of care.
• Administer medications after medication class and approval from the Administrator.
• Report to the Administrator any signs of community member abuse or witnessed misappropriation.
• Have a neat and clean appearance and adhere to the dress code as policy indicates.
• Perform other duties as assigned or delegated by the nurse verbally or in writing.
