PeaceHealth is seeking a Patient Coordinator - Trauma Program for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $21.59 - $32.39. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.
JOB SUMMARY
Responsible for the coordination of patient care for trauma acute care surgery team. Coordinates, support the TRACS team with information sharing and patient care related activities, working collaboratively with other healthcare team members.
ESSENTIAL FUNCTIONS
Assist the Primary Surgeon with a variety of patient related activities such as participating in rounds throughout the hospital, completion of daily progress notes, documentation of consults, intake History and Physical exam, and creation of discharge summaries.
Assist with new patient triage as well as answer phone calls for new patient consults when primary surgeon is encumbered.
Ensures consistent, timely and appropriate communication with patients, physicians, specialists, the emergency department and Access PeaceHealth to maintain quality care and promote patient satisfaction. Able to complete associated pertinent documentation in EHR.
Coordinates the flow of patients through the trauma program such as but not limited to the following
Activating other team members or the backup surgeon when needed.
Communication with nursing staff particularly but not limited to the OR which includes coordinating the daily OR schedule, booking cases and rearranging cases as needed.
Responsible for coordinating mobilization of the OR full trauma activations.
Ensuring the list is up to date and facilitating communication between team members.
Coordinating with consulting services, especially TRACS hospitalist services
Coordinating care of patients with outpatient clinic services, assisting with getting referrals and coordination of follow up in TRACS clinic
Ensures outpatient clinic documentation such as clinic follow up notes, history and physical, and preoperative consultations, as well as ordering of follow up studies. Oversees data entry, assist with registry maintenance and clinic duties.
Performs other non-clinical duties as assigned.
QUALIFICATIONS (required unless otherwise noted)
EDUCATION:
EXPERIENCE/TRAINING:
Minimum of two years in a lead role in a clinical or office setting preferred.
Direct patient care background preferred.
Need to demonstrate expertise in the management of workflow.
Medical terminology course or equivalent experience is required

Must have cooperative working, leadership and interpersonal skills and the ability to work effectively with a wide variety of patients, families, and staff.
Strong organizational skills.
Demonstrated insight into the roles and responsibilities of the interdisciplinary team members.

Proficient computer skills including MS Office applications and other applicable software.
Demonstrated ability to motivate others and promote positive interpersonal relationships in a patient and family centered work environment.
Ability to demonstrate a courteous, caring and understanding attitude toward patients, families, co-workers, volunteers, visitors, and physicians.
Ability to work with people from diverse backgrounds and experiences.
